Fochabers Medical Practice is currently carrying out a due-diligence risk assessment process on the new Vision Online services that allow Repeat Prescription and Online bookings to be entered directly into the Vision Medical system by patients. This represents a significant change in how we will be doing business in the future. There are a significant amount of our patients already using our web-based facilities and so the changeover should be relatively painless!
Essentially the system is more efficient and removes one layer of data entry by our clinical administrators - which in turn leaves them more time to concentrate on our most valuable commodity - You!
Access to booking your own medical appointment is a significant change in mindset and so we are exploring all of the options available to us in order to provide a sustainable and fair system for all to use.
If you would like to register as being interested in taking part in this system then click the link below and download, save, and complete the pro forma in Adobe Acrobat reader on your PC, Mac, Smartphone, or tablet.

Visit your App store for the Smartphone or Tablet. Once you have the App installed the PDF will initially show as a blank unfillable form! You must open the form on your smartphone or tablet in Adobe Acrobat for you to be able to complete the form.

If you require regular medication, we will attach a list of your repeat items to your prescription. To order your medicines, please tick the box next to each item, and post the slip in the prescription box at the health centre. You can also order your medicines by telephone, after 10:00.
Alternatively you can order online by following the link at the top of this page and completing a simple registration.
You can either collect your prescription from the surgery, or some pharmacies collect the prescriptions from us. Also, in some cases we can send your prescription to the pharmacy in pre-paid envelopes, supplied by them.
Patients on repeat medication will be asked to see a doctor, nurse practitioner, or practice nurse at least once a year in order to review these regular medications. Notifications should appear on your repeat slip. Please ensure that you book an appropriate appointment in order to avoid unnecessary delays with further prescriptions.
Please allow two full working days for prescriptions to be processed and remember to take weekends and bank holidays into account.
If you feel that you need some medication that is not on your repeat prescription form i.e. something that the GP has given you for a condition in the past, or perhaps medication which you have been prescribed recently to see if it is helpful for your condition, you will be required to email firstname.lastname@example.org stating: 'Request for Medication not on Repeat'.
Allow at least 48 hours for the GP to deal with this request.
If you cannot email the details please make an appointment with the GP. Please note that the GP may want to see you before issuing a prescription. The information we require on the e-mail request is as follows:
You can also download this document and e-mail the information if this is easier.
To use this form on smartphones or tablet devices you must install the free Adobe Acrobat Reader first. Then when you open the file in the smart phone or tablet - make sure you select 'open in Adobe'. Otherwise the form fields cannot be filled.
